How to Enroll

INSTRUCTIONS

To enroll in the ‘PIR’ study, complete the following short steps:

  1. Fill out the online questionnaire below

  2. Complete the Agreement to Participate in Investigational Research

  3. Upload your February 2020 financial statements

Once you have completed these steps, we will contact you to set up a 15-minute telephone appointment with our forensic auditor. If you have any questions, feel free to email to 'admin@pirstudy.org.' We will assign someone to call you back within the hour (or less) to walk you through it.

Step 1: Questionnaire

To participate, please fill out the online questionnaire by clicking the 'Enrollment Questionnaire' button below. Once you have submitted the Questionnaire, please return to this page to complete the Steps 2 and 3. Step 4 will be completed a bit later.

Step 2: Agreement to Participate

Once you have completed the enrollment questionnaire, click on the 'Agreement to Participate in Investigational Research' button below.

According to our institutional rules, the gathering of financial data as part of the 2018 Common Rule, the Office for Human Research Protections does not meet the definition of research requiring an IRB. The revised Common Rule at 45 CFR 46.102 states the following regarding the definition of "research":

(l) Research means a systematic investigation, including research development, testing, and evaluation, designed to develop or contribute to generalizable knowledge. Activities that meet this definition constitute research for purposes of this policy, whether or not they are conducted or supported under a program that is considered research for other purposes. For example, some demonstration and service programs may include research activities. For purposes of this part, the following activities are deemed not to be research:

(1) Scholarly and journalistic activities (e.g., oral history, journalism, biography, literary criticism, legal research, and historical scholarship), including the collection and use of [financial] information that focus directly on the specific individuals or organizations about whom the information is collected.

(2) Public health surveillance activities. Such activities are limited to those necessary to allow a public health authority to identify, monitor, assess, or investigate potential public health signals, onsets of disease outbreaks, or conditions of public health importance (including trends, signals, risk factors, patterns in diseases, or increases in injuries from using consumer products). Such activities include those associated with providing timely situational awareness and priority setting during the course of an event or crisis that threatens public health (including natural or man-made disasters).

(3) Collection and analysis of information, biospecimens, or records by or for a criminal justice agency for activities authorized by law or court order solely for criminal justice or criminal investigative purposes.

(4) Authorized operational activities (as determined by each agency) in support of intelligence, homeland security, defense, or other national security missions.

Step 3: Upload Your "Agreement to Participate..."

Once you have completed the enrollment questionnaire, click on the 'Agreement to Participate in Investigational Research' button below.

Step 4: Financial Statements

Once you have received an email notification confirming we have counter-signed the Agreement, you may then upload your February 2020 Accounts Receivable Statements by clicking on the 'Upload Accounts Receivables Statements' button below.

What's next?

Congratulations, you have completed the 4 steps of enrollment. Following the upload of your first statement, we will contact you for the following:

  • a 15-minute telephone appointment with our forensic auditor

  • 3 additional months of financial statements (one month per quarter in 2020) submitted electronically throughout the year

  • a 15-minute exit interview at the completion of the study.
